Hi there;
I just recently bought a 45 rpm record of The Beatles, featuring “Got To Get You Into My Life ” and “Helter Skelter “.
This seems to be a fairly common 45, but mine seems to have a variation. Instead of the usual photo of the early Beatles, my sleeve features the band's heads from the Let It Be cover, over a T-shirt with the British flag which reads “Beatles Forever!” I'm quite sure it's not a bootleg, as the record itself is labeled “Capitol”, as is the sleeve. Some help on identifying this would be greatly appreciated.

That cover shows that Photoshopping is as bad as scissors and glue. The great-granddaddy of bad editing. It's quite funny though.
Something tells me this was before the era of Photoshopping… so some poor sap probably had to airbrush that in somehow.

Haha yeah, long before Photoshop!
Anyways, as the address on the photo sleeve shows (3109 American Drive, Mississauga, a 10-minute walk from where I'm sitting right now) that's just a regular Canadian issue of the single – it was on Capitol but wouldn't necessarily have the same photo sleeve as the American version. I don't see any sign that it's a promotional copy – if it was, it'd say so on the label. Incidentally, what colour is the label, anyways?
GEORGE: In fact, The Detroit Sound. JOHN: In fact, yes. GEORGE: In fact, yeah. Tamla-Motown artists are our favorites. The Miracles. JOHN: We like Marvin Gaye. GEORGE: The Impressions PAUL & GEORGE: Mary Wells. GEORGE: The Exciters. RINGO: Chuck Jackson. JOHN: To name but eighty.

Not necessarily vonbontee. They could have printed the picture in Canada and shipped it down.
Is the label Capitol Canada or just Capitol? If Capitol Canada vonbontee is right but if not, I believe my theory is correct.
As if it matters how a man falls down.'
'When the fall's all that's left, it matters a great deal.

Oh-oh I didn't explain myself.
What I mean is : They did it with scissors and glue right? Well, I've seen photoshopped photos as bad as that one. So photoshopping is not an improvement at all!
(I should know this is waaay before computers, after all I majored in computer Science!!! )

That's the interesting thing; I've come across Canadian editions of this single which do have the American sleeve.
It's an orange label, which also has a strange variation, now that you mention it. Mine is on the left-hand side.
As you can see, the words “From The LP Rock N' Roll Music” are in a different location on mine, as is “Recorded in England.”
