Why did they make John McCartney Paul’s maternal grandpa when he shares the same last name and Mary McCartney was dead at the time of the movie so she would of been unable to tell Paul to look after him.

I can’t answer the former question, but as for the latter, you must keep in mind that they weren’t being fully factual with the storyline and characters. They weren’t John Winston Lennon, James Paul McCartney , George Harrison , and Richard Starkey– they were four characters named John, Paul, George, and Ringo, based upon the real people but freely changeable to suit the film’s needs.
In other words, it was a film, not a documentary.

But that hardly establishes him as the maternal grandfather. Am I missing something or forgetting a line in the script that says he is?

It was more implied by his mother saying it would do him good.
I found sources for his dad’s dad and he apparently never died. Neither did his mom’s dad. Hmmm. . . Something fishy is going on with the McCartney family.
Though his grandpa is named John McCartney but his mom’s dad is from Ireland unlike his dad’s dad.
